(4½x5½").Duplicate forms, the originals having been turned over to the Treasury Department, prescribing Whiskey and other fermented spirits for medicinal purposes. The doctor's names are illegible but the prescriptions were filled at Brenner Bros. Pharmacy at 1401 Geary Blvd. in San Francisco, now the site of Saint Mary's Cathedral. Condition: A bit of wear; near fine.
