Lot 780: Questionnaire filled out by Crawford and signed at the top, one page, 8.5 x 11, The Official Virginia Field Fan Club letterhead, no date. Crawford adds personal information such as her real name, birth date, height, weight, and education, and answers five questions concerning her film career, listing her first made film as "Dancing Daughters," her favorite film made as "Possessed," and adding that her future movie plans are "to make good pictures." Toned pieces of tape to top edge, a few tiny edge tears and creases, and toning to bottom edge, otherwise fine condition.
